SSN Letter Request Form
You must first have an offer letter of on-campus employment before you can fill out this form.
Please complete this form to receive a Social Security Letter from the International Center. This letter will be needed to apply for a Social Security Number at the local Social Security Office. Please refer to the Social Security webpage for detailed instructions on how to apply for a SSN.
Before submitting this form you must first obtain a job offer letter from your employer on department letter head, signed by the person authorized to hire you, that includes the following information: your name, department name, job title, brief job description, start date of job, end date (if applicable), number of hours of work per week, amount of pay, supervisor name and supervisor contact information.
Please note if you are applying for Curricular Practical Training (CPT), Optional Practical Training (OPT) or Academic Training, you do not need to submit this form. Once you are issued a new I-20/DS-2019 with your work authorization, you will be able to apply for a SSN using your new I-20/DS-2019 in lieu of this letter.
