COVID-19 Positive Reporting

Report positive results from an at-home test or off-site test here. If you tested positive at Campus Health and Wellbeing's health center, the result is automatically reported (you do not need to report it here). Your Isolation guidelines will populate on this page once the information is submitted. Additional information is available on the Cal Poly student COVID-19 webpage.
